Forms problem - password required

I have several forms under construction in the website in the link below,
one being www.flintanddenbighrc.com/coach-trip.htm I went to test these
forms one more time before going live with them, and they now require a
password. This box appears after pressing the submit button. This did not
happen before, can anyone help me?
