Forms with toolbars and how to remove them?

I am having to merge two databases into one and am having problems with one
of the startup forms. Somewhere it is trying to open a custom made toolbar
and I want to stop this. Is there a standard place where toolbars are
established and set to run when a form is opened and again, how do I turn it
off. Thank you!!

Open this form in Design View. Open the Properties dialog window and select
the "Form" Properties. Select the "Other" tab. Select the "Toolbar" combo
box and delete the name of the toolbar. Save the form and change to Form
View. The custom toolbar will no longer show when this form opens.
