All you need for FrontPage forms is properly installed and working extensions and correct
permissions on the folder where the form results are saved.

Also, if your form sends email, make sure that your host has enabled the form to email handler.

I’m new to this so please can you be a little more specific so to the how to.

I have XP pro SP2 IIF and the FrontPage extensions installed,

Using FrontPage wizard I created a test site, and after that I created a
virtual dir and publish the site to this virtual dir.

So far so good. I browse the site and fill the feedback form but the results
are not saved to the database (.mdb file) I have tried to change the
configuration of the web default site and the virtual dir properties I just
don’t know exactly what to change and where to change it. I hope that you and
shade some light in my direction.

For the DB functions to work you need to be working in a server based web
Are you publishing and opening your local host server root web or a subweb
(http://localhost or http://localhost/subwebname/ or http://machinename or http://machinename/subwebname/ or
