FP SP2 - Picture Icon Insert Picture + Submit Reset tags

Hi,

If I use insert/picture/from file from the menu bar then I have no
problem but if I insert a picture using the icon on the toobar then it
inserts a picture but alongside it are two tags - submit and reset.

I would like the picture icon to only insert a picture.

Any suggestions would be much appreciated,

P.S. I did run the repair but it didn't help.
