Frontpage Forms - allowing user to determine where form is sent

Hi there

I've created a 'contact us' form for my website. I'm aware how to have the
form send to a database and email address etc.

However, I'd like to have a dropdown box with several different email
addresses so users can select one of the email addresses to determines where
the form is sent.

I've thought about having multiple feedback forms but would prefer the
option of one 'contact us' page.

Any help would be greatly appreciated!
