General notes

In many of my databases I find the need for adding notes, is there anywhere I
can add general notes about things in the database. An example is query xyz
and pdq were used only for defect testing and can be deleted later. -- I
realize that some will say put this info in the title but in many cases my
notes are much longer and detailed. I would love to be able to insert a word
document somewhere inside the database.

What about a table to track database changes/mods/edits?
A date field - when, text field - what, memo field - the big picture.
