Generate flowchart from data in db

Is it possible to write code that will generate flowcharts based on data
stored in a database? and if so, is it possible to add logic that will
place the shapes with certain spacing in the Visio document so the user
doesn't have to re-arrange everything to look right? Lastly, are there
sample apps that demonstrate this?
