Generate table report in database model

I use new report wizard to generate table report in database model, the
database diagram I got from reverse engineer, the report has a couple of
sections, one is summary of columns, this table includes columns name, data
type, allow nulls and value/range, the notes of each column was displayed in
another section <column details>.
what I want is add notes as another field in summary of columns table, which
is auto generated by report wizard? Due to Visio only provide predefined
templates for report, I couldn't customize the report layout before save to a
RTF file.
