guardianship templates?

I need a guardianship template for our child who is now an adult and is
severely handicapped. She needs someone to make all decisions, medical,
financial, and daily activities of living. Where can I find one?

Consult an attorney. This is NOT something to be attempted by a layman.
Even if you succeeded in finding a template somewhere, it might not be
appropriate for the place where you live.
