Has anyone created a crime statistics or incident template?

I am looking to build a robust database about crime in my community. Has anyone
created a template or ran across one on the Internet?

I don't know one myself. If you want to find a database shouldn't you be
asking this in the Access forum?

Excel and Access are quite different products!
