When I go to View/Header and Footer, the toolbar doesn't appear. I can type directly in header and use insert page numbers, but I want all the other options available on the toolbar.

Unfortunately, you don't' tell us which version of Word you're using. But I'm guessing this could be the result of a damaged DATA key in the Registry. You should find an article on the
mvps.org/word website that explains how to reset that key.
