Help A4 template with headings and 3 colomns in landscape

Hi
I am useless at templates and wondered if some one might have one

that is A4 landscape that I can put 3 headings on then underneath between 3
and 6
columns for names and dates.

it is then going to be laminated and used as a table mat at a dinner.

any help for this old grannie would be much appreciated

Please describe in detail exactly what you need. How many rows and colums?
How large do cells need to be. I'm sure I can help but need more info.
