Help creating forms.

I have designed a form in the Compose page.
I went ahead saved and published it.
It came up just fine, but when populated and sent the email came out blank.
Do I need to create the same fields in the Read page as well??

Yes, if you have a read layout, you must put controls there if you want
people to see them. Also see http://www.outlookcode.com/d/sendform.htm for
other likely issues.
