Help find templet for gaudianship

Hi could someone help me find some kind of templet for signing over legal
rights for a child provider while out of town just in case something would
happen and my children need medical help
thanx a bunch

While it's possible that there might be a Word template available, recognize
that requirements are probably going to vary from jurisdiction to
jurisdiction. That means that you're probably better off trying to find a
