Help with Form and Tables

I am putting together a form that documents problems that
customers have with a certain product. I have the form
set up with an autolookup query so that when I type in a
business's ID number (where the customer bought the
product) the query fills in the information for that
business, such as phone, address, etc. I am now needing
to set it up to where when I type in a code for the actual
problem that the customer had, it will automatically fill
in an explanation for that code in another field. What
should I use to set up this second part of the form. Thanks
