Help with Form Design

I am in the process of creating a timesheet form. How would I create a form
with multiple rows per column for an individual record? I would like it to
look like this.

Day Time In Time Out Hours

Mon 9:00 5:00 8
Tues
Wed
Etc.

Take a look at the help files for crosstab queries. They are a bear to work
with, but it might be your only option.
