help with form

I have a basic form but it is quite large. In fact there
is over 4000 lines of HTML Code. My problem is if I run
the form without typing any information it pulls up the
confirmation page and it delivers the email notification,
but if I type any information in a field or make
selection from a drop down list, it reboots the web
server.

Is this crazy or what?!?!?!?! Has anyone had this problem?
