how can I set up a questionnaire document

I need to set up a document which will contain some text and questions to be
answered by my clients.
I would like to set it in a way that my text and my questions will be
protected (be not possible to change) and client will be only able to fill-in
answers in special fields.
How can I arrange this in Word2000 document?

It's an obscure and rarely-used feature of Word, called a "form". Display
the Forms toolbar, read the chapter in Help, and see how you go from there.

You want to create your Word document as a Form. There is a
whole series of tutorials at:
http://www.mousetrax.com/TechPage.html. Scroll down the page and
look for the series called: Please Fill Out this Form. You can
skip Part 5 unless you want to include database connectivity.

What you are talking about is what Word calls an "online form." Check this
