In Outlook 2000 and 2003 you can click on Actions tab while in a contact,
click New Letter to Contact. It should open Word and a dialog box pops up.
This is where you can Choose a Page Design. When you click on the down arrow
next to the design you should see any form/templates you have stored. Just
follow the dialog box accordingly.
