How do I add toolbar button for frequently used template?

My boss wants me to add a button to her toolbar that "you just have to click
and there's my fax cover sheet and all I have to do is fill in the
information." She doesn't want to go through the process of pulling up the
template through the menu each time.

Well...you can record a macro that opens the template with File > New.
Then put the macro on the toolbar or a custom menu.
