How do I convert a PowerPoint Slide to Excel?

I have a powerpoint slide that I would like to convert over to either an
excel or word file so that I could use it as a form.

File > Send to > Microsoft Word. Select the Outline function.

Important note: this only converts text that has utilited PowerPoint's
master template placeholders. Manually inserted text boxes will not transfer.
