How do I create a custom (non InfoPath, non-prog) form in Outlook

I want to create a simple form in Outlook 2007, not InfoPath and not using
VBA or other programming or developer skills. The link from the Forms
Overview in Outlook Help is broken. Got any info for me? Thanks in advance!
