How do I create a Fax Cover Sheet Template in Word?

I was wondering if anyone could tell me how to create a Fax Cover Sheet in
the Microsoft Word Program? If you can help please e-mail me the
instructions to [email protected] I would appreciate any and all help i
could get. I am planning on adding a company logo to this Fax Cover Sheet,
if that would be of any assistance to anyone willing to help me out in ththis
matter.

Have you looked at the fax cover sheet templates that ship with Word?
