How do I create a registration form in MS Access that can be fill.

I want to create a registration form to email out to all attendees, have them
fill it out & email back to me, then collect all of the data so that I can
use it for mail merges, tracking attendance, etc. Can someone help me? I'm
hopelessly lost.
