How do I design a questionnaire in publisher, is there a template

Hi,

I want to design a questionnaire that will go onto a printed A5 page. Does
anyone know of a tempalte I could use? The questionnaire will have free text
and check boxes and about 10 questions. Thanks

I think it likely that there are Word templates, although I do not know this
for sure. I have used surveymonkey.com successfully, in case you want to try
an on-line format.

Although they are originally designed to be "product satisfaction" an
"course evaluation" surveys, I think they can easily be adopted to you
needs by changing the questions and check boxes.
