How do I download form Format of JSY Scheme?

to apply. Plz let me know the site.

This forum is for questions about macro programming in Microsoft Word, so I
don't understand why you posted this question here.

However, using www.google.com (as you should do), I found information about
JSY at the National Portal of India site,
http://india.gov.in/citizen/health/janani_suraksha.php, which links to
http://india.gov.in/outerwin.htm?id=http://mohfw.nic.in/dofw website/JSY_features_FAQ_Nov_2006.htm.
It appears that the application format is not a national standard but has
been left to be specified by each State. It's possible that some states have
forms you can download but others do not, and probably few of them are in
Word format.
