Huh? If you mean a pedigree certificate for dogs, try asking the AKC or
equivalent in your country.
Then scan it in and use it for the background of your form (make sure to
change the fit to Stretch)
Place your controls on the form so they line up nicely with the image and
you're done. I've created
many Access forms this way when I need the EXACT representation. If an
approximation of the
form is acceptable, just enter the form yourself, coming as close to the
actual form layout as you can.
