how do I install a template with a .cab extension?

In Word, click on Tools | Options | File Locations, and note the
folder designated for "user templates". Double-click on the .cab
file and extract the .dot file from it to the user templates folder.

Not sure what you trying to save as a template, but if it can be opened in
Word and can be saved as template, simply by saving it as template.

To do this, open the document in Word, now go to File then Save As… then
under Save as type:, on its drop down menu, choose Document Template or it
may just say Template and then click save. If you want the template to
somewhere where specific, just tell it where and save it there by going to
the Save in dropdown menu on top of the Save As window.

Close Word. Open Windows Explorer, browse to the .cab file,
double-click on the .cab file, and extract the .dot file from it
to the user templates folder.
