How do i make a fax cover page

I have a hp officejet 4215 all in one fax , scanner , printer , and copier.
It wan't let me make a blank cover page for faxing that I can fill out myself.

For instance if my family want to fax someone they can fill it out using
they own name and not mines.

You don't need to use the default fax cover, you can make your own. What program are
you using?

Hi Mary, thanks for replying but this fax machaine has the obtion to use any programs to send the fax cover to but the sent to and from section filled in already . I have window XP with a hp officejet faxer it want let me edit.
