I have a weekly standing that has to be every Thursday except for the FIRST
Thursday of the month. Thanks!

Creating a non-standard appointment requires a third party add-in. The
referenced add-in should be good to go.

Warning - if you adjust each one by hand, if you ever have to change the
end date, you'll lose all your previous changes!
