How do I make special-sized rolodex cards from Outlook Contacts?

I need 2 sets. One is 4" wide x 2-3/4" high with bottom slots, other is 4"
wide x 3" high without slots. I can change paper size but there isn't a
template size that matches what I want. Apparently can't add new template
sizes?
