How do I programatically add a checkbox on a WORD Doc

I am creating a WORD document from an ACCESS form and need to put a checkbox
on the WORD doc when certain criteria is entered on the ACCESS form. I'm not
able to find the coding that does this correctly for me. Is there someone
that can assist me? Thanks.
