How do I request feilds to be completed when word launches?

I want requests to launch when word opens a certain document for users to
input things like their name and job role which update in key places like the
header and footer, the document otherwise has restrictions impossed aside
from using form fields.

The "_" signifies the code is continued on the next line, I think the
correct code is "a space" followed by "_" then "return"
