i need a daily log that has a header for company name and includes date and a
line for my daily destination. Under that i need four categories: NAME
SSN HospID Cardiologist with 8-12 lines below to enter this
information. I was able to set up what I wanted under word but I cannot bring
up the document and type name,ssn,id or cardiologist name over the underlined
spots I placed in document.
