How do I set up a policy form, where header and footer repeat on .

I need to set up a form where the header and the footer repeat on every page.
The header and footer are more complicated than a single line, and contain
information about origination date, revision date, committee responsible,
etc. When I get them to appear on every page, the body text disappears or
often only allows a few lines? I've messed with the margins, but nothing
seems to work. Thanks ...
