How do I set up a template for userids and passwords.

I need a template for userids and passwords. Impossible to remember over two
hundred and so many that have to be changed frequently. I can use the
template and save them on a CD to help me recall them. I am sure I am not
the only person in need of this.

If you use more than one e-mail address, a fourth column for "e-mail address"
would be good, too.

It doesn't need to be anything other than 3 columns - site/program, user
name, password. Why would you want a template?

Humm, then you could use column D in the Excel spreadsheet. Good idea!
