How do I set up checkboxes in excel?

I would like to have a long list of checkboxes where the user of spreadsheet
can select from the data and click an option - note: This would be a form.

The forms toolbox (Tools>Customize>Toolbars, check forms) has a checkbox you
can drag onto the spreadsheet.
