How do I setup a file upload form in Frontpage 2003?

My website is hosted through GoDaddy. I have Frontpage 2002 extension
installed on their servers.

I followed the instructions on setting up the file upload form from MS
website, however, when I publish the file and try to upload a file using the
file upload form, I get a "_derived/nortbots.htm" 404 file not found screen.
I searched and found a fix that says remove the line of code
"onSubmit="location.href='_derived/nortbots.htm';return false;".
I did this and it still did not work. Someone please help me. I've been at
this for 3 days!!!

nortbots.htm only appears in four cases:
1) You are testing on a disc-based website
2) You are testing on a website that does not have server extensions
installed or running
3) The site (or form page) was FTPed from a disc-based web, or
published using a FTP address for the destination. Publish must be to
a URL of the form http://example.com
4) There is a bug associated with FrontPage98 which causes this
problem.

From your description of the problem, I would say that the form page
was FTPed to the server. Publishing the entire web - tick the box for
a FrontPage extended site - and use a destination in the form
http://websitename.com) should fix it.
