How do I use my contacts in Outlook to roll to a fax cover sheet?

I am trying to creat a fax cover sheet where I can click a contact in
Outlook, and it will automatically populate to the fax cover sheet.

There is no menu item for a fax cover sheet in Outlook. Normally one would
create and then save a merge document for that purpose. For further
