Start by selecting a theme that's similar in some way to the one you
want to create -- either in the color palette it includes, or in the
font pair it includes (or you can start with any theme and just change
everything in it).
Now click the theme colors button to the right of the big Theme
button. Either choose one of the existing color sets, or click the
Create New Theme Colors item at the bottom of the menu and work
through the dialog to pick a different selection of colors for various
objects.
Now click the theme fonts button below the theme colors button. Either
choose one of the font pairs for headings and text, or click the
Create New Theme Fonts item at the bottom of the menu and choose a
different pair (you can use any pair of fonts that are installed on
your computer, but beware of choosing fonts that don't come with
either Windows or Office if you plan to distribute the document to
other computers).
Finally, click the Theme Effects button and choose the default effect
for SmartArt. There isn't any Create New item for this; you can only
choose from the built-in effects.
When you're satisfied with the look of your theme, click the Themes
button and choose Save Current Theme. Give it a name and let Word save
it in the Document Themes folder with a .thmx extension. The new theme
will now appear at the top of the Themes gallery in the Custom
section.
