How to change back to my old Contact interface ?

I installed a software and it change my Contact interface when entering
data. So when create a new contact, I have a totally different
interface or form. Can anyone advise me how I can change back to the
default interface ? Thanks.

But I need the software for other things. I don't know why it would
want to change the Outlook interface. I hope somebody could advise me
how to get it back. Thanks very much in advance.
