How to create a webpage to get online survey

I want to create a webpage to conduct an online survey. I like to know where
is there any available templates for it.

And also tell me whether do I need to have a good knowledge in Database or
Sever techniques.

Take a look at www.surveymonkey.com One option is to setup the survey thru
them and then in your program link to the link they provide. You don't need
