how to create multi choice answer sheets

I need to do an answer sheet where the colleague picks one of 4 answers, by
dotting the answer, so to speak.
It then needs to be able to click submit or finish, and give the toltal
percentage of right answers, and i have no idea where to start!!

If you aren't going to manually grade the results, how will you know whether
they change the answer until it's right or not?

I'd do it in Excel but they are probably other ways to handle it.

If you don't know the formulas, you're in for a hard time of it. I've been
trying to learn some of the formulas beyond the basic math functions and it
takes time. Are you sure you have the time to invest in this project? If you
do, I'd suggest posting over in the Excel newsgroups for help.

(Be aware that if someone basically writes the formulas for you, if
something fouls up the worksheet, you need to understand what's going on so
you can fix it. That's where the time comes in. Yes, this is the voice of
experience.)

You may be able to modify one of these templates
