How to get headings numbered correctly

I have a problem of not being able to get headings in documents to
number the way I want. This problem applies to Word 97, XP and 2003, on
Windows 98, ME, 2000 and XP platforms.

What I want is like this:

A Heading1 style chapter heading like 'Chapter m. Some Chapter Heading
Text' I will set 'm' as required. The number does not have to be
provided by the Style - this is a numbered heading style but one in
which the number is not shown. The alternative looks like 'm Chapter m.
Some Chapter Heading Text' - which is clumsy and looks quite
un-professional.

A Heading 2 style major topic heading like 'm.n Topic Heading Text'. In
this case I want the style sheet to generate and automatically increment
'm.n'

A Heading 3 style major topic sub-heading like 'm.n.p Topic Sub-heading
Text'. In this case I want the style sheet to generate and automatically
increment 'm.n.p'

And so on for levels 4 and 5.

I can find no way of doing this. I can come close, but then the Heading
2s do not reset after a change of Heading 1. Sometimes the Heading 3
numbers are reset, sometimes not.

Can anybody tell me how to set up the styles to achieve what I want?
