How to get my contact form in publisher to work

I have built a website using publisher 2003 and have added a contact form on
the contact us page of www.thegoldplatingcompany.co.uk. This contact form
returns with a 404 error after i type a message in the box and hit the submit
button. Just wondering what i have done wrong. Any help would be appreciated.

Your form is MIA. Gone. No trace of it. Nothing in the Source Code that
mentions form.

and even if it were there...it won't work without Frontpage Server
Extensions installed and working on your web server (they're not there
either).
