Before you include a form on your site review the form topics on the FAQ
page -

You create your form by using the form controls in the Insert menu.
Use a radio button group to offer your possible options and allow only one
selection. (not checkboxes)
You right click a form control to access the controls properties, which is
where you "program" what the control and form should do. For detailed help
with a form control use Publisher Help.
