How to make a shortcut to a form?

I have made a special holiday form. I did publish the
form to all other users. They can access the form via the
action menu, or they can choose new-use form and navigate
to the correct form.

However I want to make a shortcut so it will be easier to
access the form.

So my question is how to do this?
Where can I find the lokation where all the forms are
stored?
