how to make forms that have fields and lines that don't move

i need to make forms, either templates or other, with lines that when another
user pulls up, can go to each line and fill in information without the
underline moving, do i use (fields), templates, ex. Name: __(John
Doe)_______________
do not want to move, and do not want to remove underline

Please help - desperado
