How to set up a Student Achievement record in Access

How can I set up an Access databse to contain 350 Student exam results, but
also to include, Start Date, End Date, Student personal Achievement record,
Who their teacher/instructor is, possibly even locations of outside
attachments with duration.
I have tried to no avail. Please help!
