How to troubleshoot OWA error: "General Access Denied Error"

I've gone through multiple KB articles but I was wondering if anyone had some
experience on troubleshooting an 2003 OWA error: "General Access Denied
Error"?

What account does OWA use to access the Exchange Information store?
